Mental Health Assessment Prompts (1-5 Scale)

Stress and Resilience

  1. On a scale of 1-5, how stressed have you been feeling this past week?
  2. Rate from 1-5 how well you believe you are able to bounce back from setbacks.
  3. On a scale of 1 (not at all) to 5 (very much), how much have you been worrying about things you cannot control?
  4. How would you rate your ability to adapt to new situations, with 1 being very poor and 5 being excellent?
  5. From 1-5, how would you rate your confidence in your ability to solve problems that come your way?

Emotional Regulation

  1. On a scale of 1-5, how often do negative thoughts impact your daily activities and overall mood?
  2. How capable do you feel in managing daily stresses on a scale of 1 (not at all capable) to 5 (very capable)?
  3. Rate from 1-5 how well do you generally react to unexpected situations or changes in plans.
  4. In the past week, on a scale from 1 (not at all) to 5 (very much), how often have you experienced feelings of sadness or depression that affected your ability to function normally?
  5. On a scale of 1 (not at all) to 5 (very much), how confident are you in your capacity to manage your emotions effectively?

Cognitive Function and Focus

  1. How well can you shift your focus from one task to another? Rate this on a scale of 1 (very poor) to 5 (excellent).
  2. On a scale of 1-5, rate your ability to think creatively or 'outside the box.'
  3. Rate on a scale from one (never) to five (always): How often do you find yourself dwelling on past mistakes or failures?

Anxiety and Fear

  1. On a scale of 1-5, with five being the highest, how frequently have feelings of anxiety interfered with your daily life recently?
  2. On a scale from 1 (not at all) to 5 (very much), how often has fear prevented you from doing something you wanted to do recently?

Overall Wellbeing

  1. Using the same scale, how would you rate your ability to maintain optimism even during challenging times?
  2. How comfortable are you with uncertainty or ambiguity? Rate this on a Likert-scale of one being uncomfortable and five being very comfortable.
  3. How would you rate your current level of mental wellbeing from 1 (very poor) to 5 (excellent)?
  4. From 1-5, how often do you feel overwhelmed by the tasks and responsibilities that you have?
  5. Rate from 1-5 how well you have been coping with any personal challenges or difficulties you are currently facing.

Educational Equity and Literacy Prompts

Interconnected Systems

  1. On a scale of 1-5, how strongly do you agree that literacy and cognition are interconnected?
  2. Rate from 1-5 the extent to which you believe language can both promote inclusion and perpetuate bias or discrimination.
  3. On a scale of 1 (not at all) to 5 (very much), how much do you agree that cultural diversity impacts literacy education?
  4. To what extent do you believe that equity in education affects cognition? Rate this on a scale of 1 (not at all) to 5 (very much).

Educational Practice

  1. From 1-5, rate how effective you think various teaching strategies are in promoting diversity, equity, and inclusive language use in the classroom.
  2. On a scale of 1-5, how much do you agree that linguistic diversity influences cognition?
